Picture Day Sign Up 2026

In order to streamline the Picture Day Sign up, we have created a form for you to fill out online.  The options for space in the program for individual and group photos are listed on the form.  Once you submit your form, you will receive and email with further instructions for turning in payment.   All submissions and payments must be received by Thursday March 26th.   Your child's costume pictures can be found by scrolling down on this page to help you make a decision about how many pictures you would like to put in the revue program.   Please email Katie at katie@dancewarehouseinc.com or Chloe at dwacctmanager@gmail.com if you have any questions.

Verification Form 2026

Please fill out one form per student to verify all the information included on the form.  We use this form to verify your child's proper spelling of their name, your email address for revue tickets, and for the end of the year awards.  Please turn in by March 19TH.
